An opportunity has arisen for a Rehabilitation Worker at East Bristol Rehabilitation Centre.

East Bristol Intermediate Care Centre is a 17 bedded Rehabilitation Centre that provides a short-term rehabilitation service, maximum stay is up to 42 days, to adults who are recovering from physical injury or trauma. The aim at EBICC is to enable Service Users to maximise their independence and return to their own home safely.

Your duties will include:
  • Undertaking a Key Worker role and carry out agreed rehabilitation plans under the guidance of the multi-disciplinary team and work with Service Users on an individual basis.
  • Taking a lead role in coordinating/assisting with Service User review meetings, ensuring all relevant parties are involved and their views considered, and to co-work with other multi-disciplinary team members/other professionals to plan/coordinate timely discharges.
  • Document all observations and interventions, maintaining accurate records in line with legal requirements. To contribute to case records, in writing and electronically and contribute to the end of service report.
  • Assist in the control, recording and issuing of drugs and medicines as prescribed by the medical practitioner in accordance with directorate policies and instructions while facilitating self-medication as indicated by individual rehabilitation programmes.

Please note that this role is subject to an Enhanced DBS Check.
