Social Worker 1 or 3 – Integrated Cluster Network Team – West

Job Description

37 Hours per week

An exciting opportunity has arisen for Social Workers to apply to work with Adults in the West Network Cluster Team within the County Borough of Bridgend, working mainly, but not exclusively, with older adults affected by frailty disability and chronic ill health. Bridgend is committed to their staff and offer a strong career progression and valued training. We are looking for Social Worker practitioners with experience in a range of areas, who are dynamic and motivated to promote the wellbeing of the individuals we support in our Bridgend Communities.

In line with Bridgend’s vision of “One Council working together to improve lives” our Wellbeing Assessments involve carrying out quality strength-based assessments and providing practical, creative and innovative services. We aim to provide a “team around the person” which enables individuals to have the right support at the right time. This places the person at the centre of their care with a clear expectation that the person’s voice and choice will be heard. This exciting role will be key by working with GP practices to try to keep people in their homes as long as possible.

With the support off the Team Manager and Senior Practitioner you will receive professional support, advice and assistance with developing your existing knowledge and skills to develop a career pathway to deliver excellent Social Work practice. The model we apply is a holistic person centred strength based practice with positive risk taking.

You will be supported by District Nurses; Occupational Therapists; Physiotherapists, Speech and Language therapists, Dieticians; Pharmacists and BAVO within the Cluster Networks.

You need to be a Registered Social Worker who is confident and competent in a wide range of social worker skills, knowledge and possess high values of Social Work.
