The North Ceredigion Primary Care Cluster is seeking to appoint an enthusiastic and innovative experienced Pharmacist with excellent clinical and communication skills to join our new multidisciplinary Chronic Pain team and work alongside the Hywel Dda University Health Board Pharmacy Pain Team. Although the vacancy is for 37.5 hours the role is flexible and we would welcome applications from individuals who wish to work part-time hours.

The North Ceredigion Cluster is one of the seven Primary Care Clusters in Hywel Dda University Health Board and is located in Ceredigion, which is mainly a rural and coastal area, with Aberystwyth, a University town situated at the heart of the cluster. Geographically it expands from

Aberaeron to Borth and across to Tregaron, and includes 7 GP practices and a General Hospital. North Ceredigion are an innovative Cluster who are always looking at new and novel ways to improve patient care in their communities. They have commissioned several worthwhile projects in

recent years and are delighted to be able to work alongside the established secondary care Chronic Pain Service to develop this new and exciting Chronic Pain team in their Cluster.

As a Pharmacist within this team, you will also be responsible for supporting and advising the Cluster GP Practices to review patients with chronic pain with the aim of reducing risk, improving quality and promoting cost effective and prudent prescribing, whilst working in a holistic, bio-psychosocial team. Education and learning is another key element for the team and you will be expected to deliver training sessions to GPs, pharmacists, pharmacy technicians and other health care professionals when needed.

delivering the best possible care for the people of Ceredigion, whilst promoting innovation, research and values based health care in all that we do. Although experience in Primary Care or Pain Management is not essential, possessing the enthusiasm to work autonomously across both primary and secondary care is essential as is the desire to develop in an evolving field. The Medicines Optimisation Team will provide support when needed.

  • Four main hospitals: Bronglais General in Aberystwyth, Glangwili General in Carmarthen, Prince Philip in Llanelli and Withybush General in Haverfordwest;
